The term acne is used to describe a condition which may include comedones (blackheads and whiteheads), red pimples, and even large nodules. The condition affects mainly the face, but may appear also on the shoulders, back, chest, arms, and buttocks. As you probably know, acne is commonly a skin disease of adolescence, but it can affect adults as well. The tendency towards acne often, but not always, runs in families. Treatments which help teens can often help adults as well. Acne is treated by Dr. Gul Zikira for patients in Milpitas, San Jose and San Francisco to enhance their image and improve their self-confidence.

The cause starts with the sebaceous glands that connect to hair follicles. These glands of the skin secrete an oily material called sebum, which normally softens and lubricates the skin. The sebaceous glands become most active at puberty, leading to somewhat oily skin. Plugging of the hair follicles with dead skin cells also occurs in acne. When there is no inflammation, this leads to closed comedones (whiteheads) or open comedones (blackheads). The black color is not due to dirt, but rather from chemical changes in the cells. Finally, a bacteria called Propionibacterium acnes thrives off the sebum, overgrows, and attracts your body's immune cells, leading to inflammation and red bumps or nodules. Acne treatments are performed by our doctor for patients in our region to restore their complexion.

In most patients, acne tends to subside late in the teens to early twenties. Some patients continue to experience acne throughout adulthood. It is impossible to predict when acne will disappear. While the condition is active, it may alternately erupt and then improve. It is up to you and our physician, working together, to help control it.

Always consult with our physician or staff before making any changes to an existing skin care regimen. Look for products that work at the cellular level to re-establish the functioning of normal skin:

      • Tretinoin is the gold standard and Retinaldehyde is a newer vitamin A derivative. Products containing these molecules are effective in treating fine wrinkles, dark spots, or rough skin on the face caused by damaging rays of the sun. They work by lightening the skin, replacing older skin with newer skin, and by slowing down the way the body removes skin cells that may have been harmed by the sun.
      • Hydroxy acids are naturally occurring acids in certain plants and fruits. They work by stimulating dead cells on the surface of the skin so they can slough off more easily. A new layer of skin with a smoother texture and more uniform color can be revealed. These acids also encourage the production of connective tissue, lessening fine lines and wrinkles.
      • Anti-oxidants are important, especially vitamins C, E and green tea. They fight free radical damage and collagen break down caused by the aging process, sun damage, stress and pollution.
      • Growth factors are the newest ingredients to help your skin appear younger. They stimulate new collagen growth.

      Chemical Peels

The skin is the largest organ in our body. It is made up of millions of cells that protect us from the environment and regulate our body temperature. Every day thousands of cells die, flake off and are replaced by new cells from beneath the skin. As we age, this process begins to slow down. Our skin may become dull. Chemical Peels are performed by Dr. Zikira for patients in our region to remove dark blotches, fine lines and other signs of sun-damage and aging begin to appear.

Chemical Peels have been used for over 100 years to create an even and controlled shedding of damaged skin cells. Each year thousands of chemical peels are performed in the United States. A chemical peel uses a solution applied to the skin to remove dead skin cells and stimulate the production of new skin cells. Our doctor performs chemical peels for patients in our region to tighten skin, reduce wrinkling and restore a more youthful appearance. During your consultation with our physician you will want to explain the level of skin improvement you hope to achieve. Your surgeon will then review your medical history and examine your skin type and pigmentation to determine if you are a good candidate for a chemical peel. Together you can evaluate the advantages and disadvantages of the different types of chemical peels, discuss your options, your expectations and determine the best treatment for you. Don't forget to mention any history of viral infections, sensitivity to the sun, or prior skin treatments such as dermabrasion or Accutane treatments within the past 6 months.

To learn more about Chemical Peels, please contact our office by calling (408) 946-9453 or emailing

After our doctor performs a chemical peel, reactions are similar to a sunburn and range from mild to more pronounced. Generally, the deeper the peel, the longer the recovery. A light peel usually results in some redness, and possibly some scaling for a few days. Medium and deep peels can result in swelling and blisters that break open forming a crust and turning brown, followed by peeling which can last up to 2 weeks. These peels may require dressings on some or all of the treated skin. Dressings are usually removed after 1 - 2 days. Ointments and creams will be applied to keep the skin moist. At first the new skin will have the color of a significant sunburn. This will gradually fade to pink over the next several weeks. Cosmetics can be applied after about 2 weeks. Following a chemical peel, your new skin will be tighter, smoother and may be slightly lighter than it was before surgery. Following any skin peel, it is important that you avoid any exposure to the sun. Your new skin is very sensitive and susceptible to injury. Always apply a wide spectrum (UVA and UVB blocking) sunblock of at least SPF 15 twenty minutes before you go outside. Your surgeon will prescribe a proper home skin-care treatment program to ensure proper healing.

Treating wrinkles with Botox Cosmetic is a simple and safe procedure and the entire treatment takes about 10 minutes. Dr. Zikira performs Botox Cosmetic treatments using a tiny micro-needle. A very small amount of Botox Cosmetic is injected, very precisely, into several locations on your face. Because the needle is so fine and only a small amount of liquid is used, the pain associated with the treatment is minimal. Most patients compare the sensation to a bug bite. No sedation or local anesthetic is required, and you can resume normal activities immediately. You can even drive yourself home or back to the office. You may experience some slight temporary bruising at the treatment site. This can be covered with make-up.

      Collagen Replacement

So, you looked in the mirror and saw your first facial wrinkle! Welcome to the crowd. Baby boomers are reaching their 50s. They are more concerned than ever with maintaining a youthful appearance. And, the number of cosmetic surgery procedures performed continues to soar, growing at a rate of over 20% each year since 1996. Facial wrinkles and creases begin to appear as the underlying collagen and elastin fibers begin to break down and wear out. While this breakdown is part of the natural aging process, it can also result from excessive squinting, frowning, smoking and smiling.

The most frequently performed procedure for filling facial wrinkles and frown lines are collagen treatments. Collagen Replacement Treatments are performed by Dr. Gul Zikira for patients in our region to supplement your skin's own collagen and smooth out wrinkles and most scars. We can also improve the appearance of deeper frown lines and nasal furrows. Collagen injections are a safe, effective, time-tested, non-surgical wrinkle treatment that provides immediate results.

      Frequently Asked Questions

How are Collagen Replacement Treatments Performed?

Collagen Replacement Treatments are sometimes referred to as a lunchtime procedure, and usually take less than 20 minutes of your time. Injections generally take place in a doctor's office one month after you are tested for an allergic reaction and require no recuperation. The Zyderm and/or Zyplast collagen is injected through a tiny needle under your skin, beneath the wrinkle, into your own collagen. Several injections may be required, depending on the depth and length of the wrinkle. Because it contains a small amount of lidocaine, patients experience minimal discomfort.

How Many Collagen Replacement Treatments Will I Need?

Dermal defects are typically treated in 2 - 3 treatments over a 3-month period with correction lasting up to 2 years. Since aging is a continuing process, treatments for age-related wrinkles will be an ongoing process. Most patients come in 3 - 4 times a year for touch-ups.

      Why Choose Collagen Replacement Treatments?

      • The treatments are non-surgical.
      • Zyderm & Zyplast have a natural feel.
      • There is no need to harvest tissue from another site on your body.
      • Zyderm & Zyplast can be used alone or in conjunction with other facial rejuvenation procedures, such as laser skin resurfacing, chemical peels or facial surgery.

Which Areas Respond Best to Collagen Replacement Treatments?

The ten areas most frequently treated with collagen are: frown lines between the eyes, acne scars, cheek depressions, smoker's lines around the mouth, Marionette lines at the ends of the mouth, worry lines in the forehead, Crow's feet at the eyes, deep smile lines, dimples, and facial scars.

What Can I Expect After My Collagen Replacement Treatments?

After Dr. Gul Zikira performs your collagen injections, normal activities can be resumed immediately. Discomfort is minimal. The treatment site may have a little swelling, which may last a few hours. There may be some slight bruising which can be covered with make-up. Depending on the area, treatments usually last from 3 - 6 months. Allergic reactions are rare, but may consist of prolonged redness, itching, and/or firmness at the injection site. If you would like to get rid of your wrinkles but aren't ready for surgery, or if you would like to enhance or prolong the results of a surgery, Collagen Replacement Treatments might be the answer.

We have all known someone suffering the red-faced, acne-like effects of rosacea. These often cause significant psychological, social and occupational problems if left untreated. Rosacea is a common inflammatory disease of the face – particularly affecting the nose, cheeks, chin, and forehead. In its early stages, it most often causes red pimples and pus-filled cysts similar to those seen in ordinary teenage acne. Rosacea is a long-term disorder and usually lasts for at least several years.

Typically, flare-ups alternate with periods of less activity. Over a period of time, it gradually “scars” the skin by producing permanent redness of the face, particularly the cheeks and nose. The exact cause of Acne Rosacea is not known, but it is thought to be similar in some respects to teenage acne. It is definitely known not to be contagious. The skin disease may, in rare cases, be associated with an inflammation of the eyelids known as blepharitis. Acne Rosacea is treated by our doctor for patients in our region.

      • Redness on the cheeks, nose, chin or forehead.
      • Small visible blood vessels on the face.
      • Bumps or pimples on the face.
      • Watery or irritated eyes.

To learn more about Rosacea, please contact our office by calling (408) 946-9453 or emailing

If Acne Rosacea is fairly severe, our physician may prescribe antibiotics to help control the inflammation. All of these drugs have certain things in common: 1) They must be taken with great regularity to be of maximum benefit. 2) Though quite safe and relatively free of side effects, any disturbing symptoms which arise during their use should be reported to us so that we can determine whether they might be due to the medicine. 3) Any of the antibiotics prescribed should be discontinued immediately if pregnancy occurs. Occasionally, women taking antibiotics may develop vaginal yeast infections. Symptoms of this are itching and vaginal discharge. This is not serious or dangerous and can usually be easily managed without interrupting treatment of the Acne Rosacea. If you experience these symptoms, contact the office for instructions.

      Wrinkle Fillers

You wake up one morning and look in the mirror. Suddenly, you start noticing the development of facial lines and wrinkles. Some of the lines are dynamic wrinkles and are caused by contractions of the delicate underlying facial muscles every time you smile, laugh or frown. These lines can give a person an angry, tired or sad appearance - even when they are not experiencing that emotion.

      Frequently Asked Questions

      What Causes Wrinkles?

Facial wrinkle creases begin to appear as the underlying collagen and elastin fibers begin to break down and wear out. While this breakdown is part of the natural aging process, it can also result from sun exposure and excessive squinting, frowning, smoking and smiling.

What is the Difference Between Botox® and Wrinkle Fillers?

Botox® actually prevents dynamic wrinkles from appearing by relaxing the underlying muscles so they don't contract. Fillers are used to treat the appearance of existing folds and wrinkles by adding volume. However, used together, the results of both can be prolonged and enhanced. Wrinkle treatments with Botox® and/or fillers are among the top five cosmetic procedures in the world. Injectable fillers are effective, time-tested, non-surgical wrinkle treatments that provide immediate results. The growing list of injectable wrinkle fillers contains: fat grafts (from your own body), bovine and human collagen-based fillers, hyaluronic acids, hydroxylapatite, polymethylmethacrylate (PMMA), and poly-l-lactic-acid. Microdroplet, liquid silicone is another injectable used in some practices. Please consult our physician as to which of these products are most appropriate for you.

How are Wrinkle Filler Procedures Done?

Filler injections usually take less than 20 minutes of your time. Filler injections are usually associated with some mild discomfort which is lessened with the use of a topical or a local anesthetic. These products are injected into or under your skin, beneath the wrinkle or fold, using several injections with a small needle. The number of injections depends on the depth and length of the wrinkle.

What Can I Expect After My Wrinkle Filler Treatment?

After Dr. Zikira performs filler injections, most normal activities can be resumed immediately. The treatment site may have swelling, which may last a few hours. There also may be bruising, which can usually be covered with make-up, over the course of a few days. Depending on the filler material used and the area treated, correction usually lasts from 3 months to a year or more. Allergic reactions are rare, but may consist of prolonged redness, itching, and/or firmness at the injection site. If you have been wondering how to get rid of your wrinkles but aren't ready for surgery, filler injections may be the answer.

Are Fillers Effective for Making Your Lips Bigger Too?

As people age, there is a tendency for lips to lose volume. Another common use of filler products is to help restore lip volume. This can be subtle with lower volumes used or more pronounced if you desire.

      PDO Threadlift

As we age, gravity and muscle action create changes in the tension and elasticity of facial skin. This leads to sagging skin, wrinkles, creases and deep grooves around the mouth and in the forehead. Many of our patients are concerned with correcting these outward signs of aging as well as other physical imperfections in the facial area. For many patients treatment with soft tissue fillers, such as collagen, Restylane® and Radiesse®, can be inefficient and require repeat treatments. This is because these patients need more than soft tissue replacement, but are not ready for a full facelift.

A new, more economical, non-surgical procedure called a PDO Threadlift is performed by Dr. Zikria for patients from Milpitas, San Jose, Sunnyvale to give a lasting lift to sagging facial tissues without the expense and prolonged recovery of a full facelift. The procedure is safe, simple, conservative and leaves no visible traces of the procedure on the skin. It can make a patient look 5 - 7 years younger.

The PDO Threadlift uses a patented barbed suture made of polypropylene. The unique feature of this suture is the hundreds of tiny barbs that are laser cut all over its surface. The procedure is done using local anesthesia and can take from 30 minutes to 1 hour. Dr. Zikria gently threads the suture through your sagging tissues and then retracts it. This gentle retraction causes the barbs to lock onto and lift the sagging tissues, allowing our doctor to reposition them under the skin. Once in position, your body begins generating new collagen around each barb to hold them in place.

Following a PDO Threadlift downtime is minimal. You probably won’t even need bandages. However, you may want to apply some ice to reduce swelling. Most patients resume full normal activities in 2 - 3 days.

To learn more about PDO Threadlift, please contact our office by calling (408) 946-9453 or emailing
