- Fully Tummy Tuck - Involves an incision around the belly button and another incision that runs from hip to hip. Your entire abdominal wall is tightened with permanent sutures, and the excess skin and fat are removed.
- Mini Tummy Tuck - Usually performed in conjunction with liposuction of the upper abdomen and the flank areas. With a mini tummy tuck, a smaller incision is made in your bikini area and no incision is made around your belly button. Our surgeon will remove your excess skin and fat through this incision. Permanent sutures will also be placed through this incision to tighten the muscles of the abdominal wall.
Regardless of which procedure you have, your incisions are closed with small sutures. Small drains may also be inserted to eliminate fluid buildup. Snug elastic bandages are then applied to the area. After Dr. Zikira performs your tummy tuck, bed rest is recommended for the first day. Most patients experience some numbness in the area of their abdomen, which may last as long as a year. Any pain or discomfort that follows surgery can be controlled with oral medication. If drains are used, they may remain in place for 3 - 7 days until discharge fluids decrease. Sutures will be removed in 1 - 2 weeks, however, your bandages may remain a little longer. A special abdominal support garment is often necessary for several weeks. There will be some scarring that will fade significantly over time. During your recovery, you must refrain from any strenuous activity or lifting. The implant is securely positioned in the pocket, and small sutures are used to close the incision. When inflatable implants are used, your surgeon can adjust the amount of inflation. There is no evidence that breast implants increase the risk of breast cancer or any other condition. Most women can also safely breastfeed after augmentation surgery.

Cosmetic breast surgery has become one of the most popular cosmetic surgeries in the world today. The vast majority of cosmetic breast procedures are for augmentation. However, breast lift procedures are performed by Dr. Zikria for patients in our region to rejuvenate the appearance of their breasts by providing a younger looking, more comfortable lift. Most people think a breast lift is requested more often by older women. However, the average age of patients choosing to have the procedure is only 39. This is just slightly older than those having augmentation and reduction procedures. As women age and/or have children, their breasts tend to lose fat and the breast tissue begins to thin. In most cases, when the breast tissue shrinks, the skin does not and the areola/nipple droops down below the inframammary crease. Lifting the nipple above the crease will restore a youthful look. While it is possible to nurse after a breast lift, it is generally recommended that you wait.
Should you become pregnant your breasts will swell and stretch the skin and you may lose the benefits of the procedure. Many patients who request a breast lift also want to have the fullness back. These patients usually decide to have implants as well. In certain instances, the augmentation will aid in stretching the skin, enabling your surgeon to do a breast lift with shorter scars. Other patients may require a breast reduction as well as a breast lift to restore the look they desire. Breast lifts are performed on an outpatient basis using general or local anesthesia. A breast lift may take 2 hours depending on the complexity. Your surgeon may choose from a variety of surgical approaches. The most common approach, the "anchor" incision, is actually three incisions. The first incision is made around the areola. The next runs from the bottom of the areola down to the breast crease and the third incision follows the horizontal breast crease. Working through these incisions, excess skin is removed from the lower portion of the breast. Areolas that have been stretched over time can be resized and the tissue is repositioned to reshape the breast. The breast tissue and nipple are lifted into the higher position, and the nipple is repositioned. Like many surgeries, the trend in breast lifts has been to smaller, shorter incisions resulting in less scarring.
These smaller scar techniques include the "lollipop" incision which avoids the horizontal incision beneath the breast and the "peri-areola" or "donut" incision around the areola which avoids both the vertical incision from the breast crease to the areola and the horizontal incision beneath the breast. After examining the size, shape, and sagging of your breasts and the size of your areolas, your surgeon will discuss the technique best suited for your procedure. The size and amount of scarring are different for every patient. However, the use of silicone sheathing, topical liquid silicone or laser treatments can greatly diminish the appearance of scars. Bed rest is recommended for the first day. For the first few days following any breast surgery, you may be asked to wear surgical dressings or a special post-surgical garment. Breast Liposuction Reduction
Not all women think having large breasts makes them more attractive. In fact, many women are eager to reduce the size of their breasts to ease the discomfort, posture problems or the embarrassment. For them, just the thought of squeezing into tight clothing, dancing or playing sports can be a nightmare. Breast reductions are performed by Dr. Gul Zikira for patients in our region who are concerned about the effects the excess weight of their breasts will have on their upper body. In the past, having the size of your breasts reduced meant undergoing surgery that could take up to 8 hours, several days in the hospital, followed by extensive recuperation, significant pain, and extensive scars. Now, Dr. Zikira performs Tumescent Liposuction to reduce breast size a few sizes with almost no visible scars. It is done under sedation with local anesthesia in less than 3 hours. Patients go home the same day usually with only minimal discomfort. Some patients resume work in about 1 - 2 days and are back to full activity, including sports and dancing, within a week or two.
The best candidates for this procedure are women who desire a reduction of only a few cup sizes in the size of their breast. Breast asymmetry may also be corrected using this technique. Often a mammography (X-ray) may be required. With breast lipo-reduction, we can only reduce breast size a few cup sizes and we can not guarantee the size and exact shape of your breasts following the procedure. In most cases, the height of the nipple will come up about 2 cm and the size of the areola will shrink in proportion to the smaller breast size. Dr. Zikira performs breast reduction by liposuction on an out-patient basis. It is performed using light sedation and tumescent local anesthesia. Several tiny, well-concealed incisions are made in the breast fold, to insert the cannula. The fatty tissue is gently suctioned away. The entire procedure takes three hours or less. Post-operatively, most patients experience minimal discomfort. They are only in the recovery room for about two hours and then they go home. Dressings are worn for the first day or two. Patients are encouraged to resume normal activities as soon as they feel up to it. The results of breast reduction surgery are usually permanent but breast size may change due to weight gain or pregnancy. Breast reduction can make a dramatic change in your appearance and your outlook. A breast reduction can make your breasts smaller and firmer. Many patients find new enjoyment in sports, exercise, dancing or even just walking.

Brachioplasty (Arm Lift)
Brachioplasty, commonly referred to as arm-lift, is a surgical procedure performed by Dr. Zikira in San Jose, that reshapes and improves contouring of the arms by reducing excess sagging skin that hangs downwards and reduces pockets of fat in the upper arm area. The surgery entails the tightening and smoothing out of the underlying supportive tissue that defines the shape of the upper arm. Drooping of the upper arm skin is usually due to weight fluctuation, old age, and/or genetics. A candidate for brachioplasty is someone who has lost excessive weight and consequently has extra skin in the upper arm region. Extra skin does not respond to diet and exercise.
Depending on the excess skin and fat of your upper arms, the physician can perform three of the following procedures:
- Liposuction of the upper arms, which is the least invasive option. The scar is practically not visible as it is located under the axilla (armpit). This option is usually for patients who have excessive fat, not skin.

Lower Body Lift
Do you remember what you looked like before time and gravity took over? Millions of men and women diet and exercise on a regular basis. Unfortunately, many of them end up disappointed by the loose, sagging skin remaining on their thighs caused by a significant weight loss. Others find that no matter how hard they work out, they can't reverse what time and gravity have done to their buttocks. The hips, thighs, stomach, and buttocks are prone to excessive fat accumulation. They often contain deposits of fat that do not respond to diet and exercise. Thigh, hip and buttock lift surgery is performed by Dr. Zikira for patients in our region to tighten the skin of the outer and inner thighs, hips and buttocks.
This procedure is often combined with a tummy tuck (abdominoplasty). These procedures improve the firmness of skin and underlying tissues. For those with tight, unstretched skin, liposuction alone is the treatment of choice. If the skin is overly stretched, loose and flaccid, liposuction will be combined with excisional surgery to remove the excess skin along with the fat. Men and women with loose skin in the outer and inner thighs and with sagging buttocks are the best candidates for thigh and buttock lifts. Liposuction is the most common cosmetic surgical procedure performed today. Liposuction surgery is performed to actually remove unwanted, localized deposits of fat cells from the body. The procedure is meant for body contouring, not for treatment of obesity. A suction tube is inserted through a tiny incision placed in an inconspicuous location on the skin. With a high-pressure vacuum, fat cells are extracted through this tube. The result is a resculpting of bulging areas into more attractive contours. Once removed, the fat cells are gone forever. Excisional surgery is combined with liposuction when the patient has excessive folds of skin. An incision is made and concealed in the groin area and under the buttocks. The skin is tightened, redraped and the excess skin is removed. Sutures are used to close the incision.
After Dr. Zikira performs your lower body lift, you will go home and rest for at least 24 hours, after which you may gradually increase your activity as tolerated. Some discomfort and pain are to be expected, but it can be controlled with medication. The bandage over the incision site will be removed the day after surgery and you will be able to shower. Generally, it is necessary to wear a light compression garment 2 - 3 weeks after your surgery. All sutures are internal and absorbable. After 2 or 3 days you may resume most non-strenuous activities. Walking and other non-strenuous activities including work may be resumed after a week or two. Sports and other strenuous activities are prohibited for at least two months.
During the first 10 - 14 days, you will be bruised from your upper thigh down. Scars will be red for the first few months, but they will gradually fade. Patients who have had liposuction will have more swelling than those who only had a lift performed. The use of smaller cannulas during your liposuction allows the fibrous connections between your skin and body to remain. These connections will contract over time, after which the excessive skin folds will be gone.

Not long ago, a facelift was considered by many to be reserved for movie stars and celebrities. Today, a facelift, or rhytidectomy, is not just for the rich and famous. It is one of the most well accepted and most frequently performed cosmetic procedures in the United States. Recent studies have shown that looking good has a direct correlation to higher pay. Facelifts are performed by Dr. Zikira for patients in our region so that they can look younger and feel better. People from every walk of life elect to have a facelift for cosmetic as well as economic reasons.
If you have been considering a facelift to reverse the signs of aging, you should understand what a facelift can and can't do. Although the incisions made for a facelift are hidden back in your hairline, it does nothing to correct the visible signs of aging in the upper face. A facelift provides an overall lift to the lower third of your face by tightening loose skin in the jowls, neck, and jawline. A facelift alone will not remove forehead wrinkles, crow's feet or bags under your eyes. These areas can, however, be treated at the same time as your facelift. During your initial consultation, your surgeon will discuss your particular condition, your expectations, and the probable outcome of your various treatment options. You will be shown several before and after photographs of patients having similar conditions, and we will explain the details of your procedure. A facelift procedure may be performed in your doctor's office or in an outpatient surgical facility. Premedication is generally recommended to relax the patient. The procedure is usually performed under general anesthesia with the patient asleep but can be performed using local anesthesia. Your surgeon will operate on one side of the face at a time. The incision is placed in the natural crease of the facial skin and begins inside the hairline, in the temple area above and forward of the ear. It continues inside the ear, or in front of the ear around the earlobe and behind the ear. The skin is lifted outward and the surgeon tightens and repositions the underlying muscles and tissues. Excess skin and fat may be removed. The incision is then closed with fine sutures and/or metal clips. If metal clips are used, shaving hair from the incision site may be avoided. A facelift can take between 2 - 4 hours depending on the extent of the procedure. Some relaxation of facial tissues will occur during the first few weeks. After that, the aging process continues. Your lifestyle choices, bone structure, skin type, sun exposure, and heredity all play a role in determining how many years can be removed and for how long. In general younger patients enjoy longer-lasting results. If you are in your forties, the effects may last up to 10 years. If you are in your sixties, your results may last 5 - 7 years.
After Dr. Zikira performs your facelift, there will be bruising and swelling that will persist for a few days. Pain is normally minimal and can be controlled with oral medication. We will take off your bandages in our office in 1 - 2 days after surgery. You may then go home, take a shower and wash your hair. Sutures may be removed in stages to minimize scarring.
Healing is a gradual process. After about 2 weeks you will feel comfortable (with make-up) among strangers. After 3 weeks you will look and feel confident enough to go out with friends again. Your face will feel completely "normal" in about 10 - 12 weeks. Direct sun should be avoided for several weeks. Most patients are delighted with their new, more youthful and relaxed appearance.

Short Car Facelift or S-Lift
Your face provides the world with a lasting impression of who you are and how you feel. But, it doesn't have to tell your age. For many of our patients, the "wind-tunnel" look caused by overly taut skin following a facelift is no longer acceptable. They desire a more "natural looking" rejuvenation. Busy patients demand more immediate results with little, if any, downtime, smaller incisions, no visible scars and no potential for hair loss. The S-Lift, or short scar facelift, is performed by Dr. Zikria for patients in our region to provide an overall lift to your neck and the lower third of your face. Compared to a traditional facelift, it is a quicker operation with fewer complications and a much faster recovery. The S-Lift is particularly well suited for middle-aged and younger patients and can make a dramatic change in the way you look by tightening sagging muscles and skin of the cheeks, chin, and neck. An S-Lift is performed using tumescent local anesthesia and may be combined with facial and neck liposuction. Following surgery, the normal aging process resumes. A browlift can be performed as an outpatient procedure under local anesthesia with deep sedation, or general anesthesia, and may take 1 - 2 hours. With the classical or "open" method the incision is hidden just inside or by the hairline across the scalp and between the ears. Your surgeon lessens the appearance of wrinkles by removing selected muscles and releasing tissues, which cause the wrinkles. The eyebrows may also be elevated, and excess skin at the incision will be removed to allow a gentle redraping of your forehead skin. An endoscopic or minimal-scar browlift is performed using a tiny fiberoptic lens inserted through half-inch incisions placed in the hairline. The lens is attached to a camera, which is connected to a television, allowing for visualization of the surgery. Additional half-inch incisions are made for the insertion of tiny scalpels, scissors, and retractors. You may wear make-up after 3 days, and your sutures will be removed in 7 - 10 days. Strenuous activity is restricted for 6 weeks.

As we age, different areas of the face and neck are affected differently. Some people develop wrinkling and sun damage in their entire face and neck. Some people seem to lose facial fat, while others accumulate fat in the neck and maintain good skin tone. Others may develop more exaggerated signs of aging in their neck. However, it is important that you continue to wear the elastic garment at night for 2 weeks. Any sutures that require removal are taken out after about a week to 10 days. Over the next several months, as the healing process occurs, the soft suppleness of the neck skin will return. Blepharoplasty is eyelid surgery that is performed to improve the appearance of the eyelids. Depending on the patient it can be performed on the upper lids, lower lids or both. Each year thousands of women and men choose eyelid surgery to help restore a more energetic and youthful appearance. Eyelid surgery, or blepharoplasty, is performed by Dr. Zikria for patients in our region to correct sagging eyelids, remove fatty pouches beneath the eyes and remove excess skin from the upper eyelids. Blepharoplasty can be done alone or in conjunction with other facial surgery procedures such as a facelift, a brow lift or laser skin resurfacing.
During your initial cosmetic consultation, your doctor will discuss your particular condition, your expectations, and the probable outcome of treatment. Blepharoplasty can be performed in your doctor's office, an outpatient surgical facility or a hospital. It is generally performed under local anesthesia but can be performed under general anesthesia. The surgeon places fine incisions in the crease above each eye to correct problems of the upper eyelids. Then a thin crescent of excess skin and muscle is removed. The incisions are closed with very fine sutures. To remove the fatty deposits beneath the eyes, the incisions are placed just inside the lower eyelid. After the fat is removed, the incisions may be closed with absorbable sutures or may be left to heal naturally. When excess skin and muscle needs to be removed from the lower lids, the incisions are placed in the natural smile crease below the lashline. The procedure can take between 1 - 2 hours, depending on the extent of the surgery. Eyelid problems are common, even in younger people. Correcting eyelid problems that interfere with your vision may be covered by your health insurance. However, when eyelid surgery is performed for cosmetic reasons, it may not be covered in part or in whole by your health insurance. When Dr. Zikria performs your eyelid surgery alone, and not with a facelift, it is generally done under local anesthesia. After the surgery, the area around your eye will be bruised and swollen for about 2 weeks. Patients should keep their heads up and place cold compresses on their eyes periodically. Antibiotic eye drops or ointment may be prescribed. Patients can't wear contact lenses for about 2 weeks, since the eyelids may be stiff and sore. Make-up may be worn after about 10 days. The resulting hairline scars which follow the natural contour of the eyelids tend to fade in about 2 months. After cosmetic surgery, our patients have a softer, more youthful look to their faces. Many seem to have a new spring in their step after facing their fresher reflections in the mirror.

Fat Grafting (Fat Transfer)
Facial wrinkles and creases are part of the aging process. They begin to appear as the underlying collagen and elastin fibers break down and wear out. While this breakdown is part of the natural aging process, it can also result from excessive squinting, frowning and smiling. Fat transfer, also called autologous fat transplantation or micro-lipoinjection, is performed by Dr. Gul Zikira for patients in our region to plump up facial features with a patient's own fat. It is a popular alternative to other wrinkle treatments. Fat transfer to the face is a safe, natural, non-allergenic procedure designed to recontour your face and can provide definition to your cheeks and chin. It can also be used to correct facial deformities. Since fat transfer uses your own fat cells, you can never have an allergic reaction, because it's from your own body.
With fat transfer, it is possible to have fat liposuctioned from your thighs and then transferred to any area of your body. Reinjected fat lasts longer in larger areas of non-movement, so it is very successful for the correction of sunken cheeks because this area does not move as often. Fat transfer can also correct atrophic aging of the hands and post-surgical and post-traumatic defects. But not every area of the body responds well to fat transfer. The breasts, for instance, are out. Injecting fat into the breast makes it much more difficult to detect breast cancer with mammograms.
Fat transfer is performed in our office on an outpatient basis. Both the area from which the fat is taken and the treatment site is anesthetized with a local anesthetic. Using a small needle attached to a syringe, fat is removed from a donor site where your fat is most tightly packed, such as your abdomen or your buttocks. Once removed, your fat is processed to remove excess fluids and then reinjected using another needle, which is placed just under your skin beneath the wrinkle. This process may be repeated until the desired correction has been achieved. Harvesting the fat cells for reinjection is often performed in conjunction with other procedures, such as liposuction, tummy tuck or cosmetic breast surgery. When fat is harvested during another procedure, it is often possible for your surgeon to harvest a larger amount of fat, then freeze and store your fat for future injections when they become necessary.

For results that will last years, patients usually receive 3 - 4 treatments over a 6 month period.
