      • Excisional Surgery - The cutting away of scar tissue. Punch grafting is the removal of the scar tissue and replacement with a graft of good, healthy skin.
      • Scar & Wrinkle - Release is a minimally-invasive technique (also known as "undermining" or "subcision") that breaks apart the depressed scars, deep creases, or fine lines with a special needle or instrument. New tissue is created that raises the scar or unwanted depression. It is also used to smooth wrinkles. The procedure takes about an hour.
      • Dermal Filler Injections - Good treatments for shallow and deep scars. These fillers can be injected into the area of the scar, smoothing depressions. Results are immediately apparent. Patients usually require touch-up treatments every 6 months to 2 years.
      • Microdermabrasion - Likened to a "superficial skin polishing" using micro crystals vacuumed through a delicate hand piece. In as little as 20 minutes, this gentle abrasive technique efficiently produces satisfying results. Treatments are progressive and are applied through a course of weekly sessions.
      • Chemical Peels - Use a chemical solution to gently burn away, in a controlled fashion, the fine outer layers of the skin. The solutions are available in several strengths, depending on the degree of peeling desired. Healing of the skin tissue can take from 3 days to 3 weeks. A new, smoother layer of skin is then formed.
      • Non-Ablative Laser Treatments - Not painful and there is no downtime. Most patients receive a series of about 5 treatments. Depending on the type of scar, improvement can be dramatic. The improvement is gradual and continues to improve even after the treatment stops.
      • Fractional Skin Resurfacing - Treats facial scars by delivering the laser's energy in microbeams. Fractional treatments promote the production softer, smoother skin.

      Unsightly spider veins and red blotches on the face affect millions of people. While they may not be medical emergencies, many of our patients would like to be rid of these unattractive and sometimes embarrassing distractions to their natural beauty. Fortunately, facial vein treatments are performed by Dr. Gul Zikira for patients in our region and can safely, easily and effectively treat these facial spider veins, also called telangiectasias. Prior to treatment our physician will review your medical history to determine how long you have had a problem with facial veins.       Today's new light and laser technology delivers precise dosages of energy to each vein, with a minimal risk to the skin. During these treatments, light energy is delivered through a special handpiece to the targeted vein. The energy is then absorbed by the blood vessels. The heat/energy absorption causes a thermocoagulation (clogging) of the blood vessel. The clogged vessel is then slowly absorbed by the body. Most patients describe the sensation to be like the snap of a rubber band on the skin as the pulses of energy are delivered through the handpiece. Some patients opt to have a topical anesthesia. After treatment, the pain is minimal to nonexistent.

      Typically, multiple treatments are necessary to achieve optimal results. Tiny pink and red vessels usually require two to three treatments to achieve acceptable results. Veins that are darker or purplish are usually more difficult to remove, and require more treatments. Treatments are spaced at 4 - 6 week intervals.

      After Dr. Zikira performs your facial vein treatment, postoperative discomfort is minimal. Your postoperative appearance will vary depending on what type of treatment your doctor selects for your condition. After treatment with certain lasers or light systems some swelling, redness and on rare occasions, some minor blistering may occur. Following treatment with a pulsed-dye laser, some bruising may appear around the treated areas. This can be covered with make-up if desired and will fade away in about 2 weeks. Following any treatment, your skin may feel tender for several days. You should follow your doctor's instructions, avoid any harsh skin cleansers and use sunscreen.

      Fractional Laser

      Nothing is as beautiful as soft, smooth, healthy skin. Unfortunately, our skin is subject to numerous experiences that leave it less than perfect. Time, heredity and gravity all do their part to create fine lines and wrinkles, especially around the mouth and eyes. Continued exposure to the sun and smoking are known to significantly accelerate the development of these natural signs of aging.

      Some skin resurfacing devices, such as CO2 lasers, use the ablative method: removing the epidermis and some of the dermis. While a good result is eventually seen, the drawbacks include red, raw, and unsightly skin, and a risk of infection for several weeks after treatment. Other devices use the non-ablative method: sending light pulses into the dermis, while leaving the epidermis intact.       How Do IPL Treatments Work?

      IPL treatments are performed with a non-invasive medical system that uses intense pulsed light therapy to reduce melanin, the dark pigment in your skin.       The best candidates for neck rejuvenation are patients in good health with realistic expectations. They have good skin tone and elasticity. It is helpful if they have a fat pad under the skin and on top of the neck muscles.

      What conditions can be corrected and with which procedures?

      • Loose, sagging skin under the chin requires tightening of the skin with a laser
      • If the skin is too excessive, it may need to be removed with excisional surgery
      • If you have visible vertical bands in your neck, the neck muscles will need to be tightened in the midline
      • Fat deposits in the neck can be removed with liposuction or direct excision

      Depending on the extent of the procedure, you may have some swelling and localized bruising which can be covered with make-up. Both are generally gone within 4 - 10 days. A bandage will be placed underneath your chin and an elastic bandage is wrapped around your neck and head. The next morning you may remove the bandage. However, it is important that you continue to wear the elastic garment at night for 2 weeks. Any sutures that require removal are taken out after about a week to 10 days. Over the next several months, as the healing process occurs, the soft suppleness of the neck skin will return. After Dr. Zikira performs your neck rejuvenation procedure, your pulled back and tightened neck will have a youthful, more defined profile. You may resume normal activities almost immediately.       How does Photo Rejuvenation Work?

      Photo Rejuvenation systems deliver pulses of intense light, through very specific filters, to the skin. Once delivered, the targeted tissue absorbs the light's energy. There will be a series of 5 - 8 treatments depending on the area and the condition of your skin. Treatments take between 15 - 30 minutes and are administered in the doctor's office.

      What Results Can I Expect from Photo Rejuvenation?

      After Dr. Zikira performs each Photo Rejuvenation session, you will experience some improvement in skin tone and texture. There will be a decrease in the overall redness and flushing. Sunspots will fade; pore sizes will shrink. Fine lines and wrinkles will gradually decrease in size as the new collagen forms beneath the skin. These improvements will continue to increase with each treatment.

      Scars and Stretch Marks

      Unsightly scars and stretch marks, or striae, have presented problems for men and women for centuries. Scars and stretch marks are generally considered unsightly, disfiguring and unacceptable by many people. Scars form as part of the skin's healing process whenever multiple layers of skin are injured. The injury may be the result of an accident, surgery, a burn or even severe acne. While the exact cause of stretch marks is unknown, it is thought that striae are the result of your skin's collagen and elastin breaking down under stress. Pregnancy, body building and weight loss are the leading causes of stretch marks. Stretch marks occur most frequently in areas of mechanical stress such as the breasts, abdomen, thighs, groin or buttocks. In the beginning, stretch marks are red. They turn white as part of the healing process.

      In the past, cryosurgery (freezing), excision, steroid injections and the use of topical creams and ointments were the standard of care for the treatment of scars and stretch marks. The results of these treatments range from fair to minimal, with recurrences commonly seen after many of these treatment options. Today, laser treatments to treat a wide variety of scars and stretch marks are performed by our doctor for patients in our region. The laser's energy stimulates the skin under the scar and causes the fibroblasts in the skin to begin to divide again. It also stimulates the production of collagen. This thickens the skin under the scar until it closely resembles the surrounding normal skin. Our current laser technology has been shown to improve or eliminate red or raised scars. Recent studies have demonstrated that laser treatments stimulate the production of collagen and improve or eliminate the appearance of stretch marks. Additional studies show that the appearance of stretch marks continues to improve over a period of time ranging from 3 - 6 months after the laser treatment was given. Even those treatments that showed little if any initial improvement showed as much as a 50% improvement at their 6-month checkup.

      Note: Patients with a history of Vitiligo, pigmentation disorders, dark tans or dark pigmentation may require an initial skin test for evaluation by our physician prior to treatment. Scarfade™ and Laserfade™ are silicone gels that when applied to the skin form a micro-thin layer of silicone on the scar. They can be worn under cosmetics and are both easily tolerated by children. The silicone is thought to moisturize the skin, reduce capillary irritation and allow the skin to return to its normal texture and color. Most patients do not require anesthetic. Some patients have indicated that they feel a slight tingling when the laser pulses, or that the laser pulse feels like the snap of a rubber band against the skin.

      After Dr. Zikiraperforms a laser treatment, results vary from patient to patient and may not be immediate. Results from laser scar and stretch mark treatment are generally progressive and may require multiple treatments. If you respond to the first treatment, your second treatment will usually be as good as the first, and the third treatment will be almost as good. About 70% of patients receiving laser treatments for their stretch marks notice a marked improvement. Newer and shallower stretch marks and scars respond better than older and deeper scars and stretch marks. To enhance the results of your laser treatments your surgeon may recommend the use of tretinoin cream or a glycolic acid cream prior to and after your laser treatments.

      Spider Vein Treatments

      Spider veins, or telangiectasias, are those small red, blue and purple blood vessels that appear most frequently on the face and thighs. Varicose veins are usually larger than 1/4 inch in diameter and may appear raised with a bluish color. They may also be related to more serious vein disorders and require a careful evaluation by Dr. Zikira prior to treatment.

      Today's new medical lasers provide safe and effective non-surgical treatment options for the removal of spider and varicose veins. New, advanced laser treatments are performed by our doctor for patients in our region to deliver a precise dosage of energy to each vein. During your treatment, light energy is delivered through a special laser handpiece to the targeted vein, in a series of brief pulses. The light energy is absorbed by the blood vessels, but not the surrounding tissue. This heat absorption causes a coagulation of the blood vessel, which is subsequently absorbed by your body.

      Will Laser Vein Therapy Work on My Veins?

      Prior to treatment our physician will review your medical history to determine how long you have had a problem with your veins and the severity of the problem. We need to determine your skin type and if there has been any prior surgery or treatment on your veins. We may test to determine if there are any underlying venous problems. If there are, further testing may be advised before laser vein therapy is attempted.

      What are Laser Vein Treatments Like?

      First, we will identify the veins to be treated. We then proceed to deliver pulses of light (laser) energy along the length of the vessel, larger veins first. The light energy causes the blood within the vein to coagulate, eventually destroying the vessels, which are then absorbed by your body. Most laser therapy patients report little discomfort. Some experience a mild burning sensation when we pulse the laser.

      How Many Laser Vein Treatments Will I Need?

      The number of treatments required varies with the size and depth of each vein. For the maximum long-term resolution, multiple treatments may be necessary. However, if new veins appear, additional treatments are required. Treatments may be in 3 - 4 week intervals.

      Are there Other Treatments Available for Spider and Varicose Veins?

      Yes. Laser vein therapy may be combined with injection sclerotherapy. During sclerotherapy, a sclerosing solution is injected into the vein through a micro-needle. The tiny needles generally cause very little pain. The sclerosing solution causes the vein to blanch (turn white), then gradually disappear. A typical treatment may last for 15 - 20 minutes and consist of multiple injections.

      What is Recovery from Laser Vein Treatments Like?

      After Dr. Zikira performs your laser treatment, you may be asked to wear support hose or special heavy duty stockings to help keep pressure on the veins. This will help keep the treated veins closed. Bruises around the treated areas usually disappear after the first week. A tan or brownish pigment may persist for a few weeks when larger veins are involved. While each vein may require several pulses from the laser, most will disappear in 2 - 6 weeks. Keys to success with laser leg vein therapy include the avoidance of hot baths and strenuous exercise, and may include the short-term use of compression stockings. Patients are generally encouraged to resume most normal activities immediately. We do ask that you refrain from vigorous activities for the first 24 hours. Although your final results may not be apparent for several months, most of our patients enjoy the comfort and the look of legs without unsightly red, blue and purple veins.

      Spot Removal

      Your face, hands and arms are the first parts of the body to show signs of aging because they receive so much exposure. Sunlight, the elements and the natural aging process all contribute to the development of dark spots and freckles. Age spots and freckles are forms of pigmented lesions. They are caused by excess production of melanin in the skin. Melanocytes are the cells that manufacture melanin (the dark pigment in skin). Most of us have some form of pigmented lesion. While spots and freckles may not be medical emergencies, many of our patients would like to have them removed. Fortunately, spot removal is performed by Dr. Gul Zikira for patients in our region using advanced medical lasers that remove these distractions safely, easily and effectively.

      Today, lasers can remove 90 - 95% of the pigmented lesions effectively. Lasers and advanced light systems convert electrical energy into intense light. Different systems produce different colors (wavelengths) of light. Each color light is used to treat specific tissues. For example, the light which is used to treat lesions is brown and will not have any effect on other colored tissues.

      During the consultation with our physician, explain your goals and expectations of the treatment. Dr. Zikira will assess the nature of your skin lesions, your expectations and explain the probable outcome of each procedure. You may be shown several before and after treatment photographs of patients with similar spots, and our doctor will explain the procedure you can expect to receive and the results that you can anticipate. Most patients describe the sensation to feel like the snap of a rubber band on the skin as the laser pulses are delivered through the handpiece. Some patients request a topical anesthesia for added comfort. Following treatment, the pain is minimal to nonexistent.

      Typically only 1 - 2 treatments are all that is necessary. Occasionally darker, larger spots may require additional treatment to obtain optimal results. Laser Spot Removal gives Dr. Zikira precise control as the procedure gently removes the spots to reveal smooth, fresher-looking skin. The laser's energy is delivered in pulses via a handpiece held very close to the skin. As the laser pulses, you may feel a slight sting. If needed, a local anesthesia can be applied to the treatment area. Spots are removed in a precise, rapid, bloodless fashion achieving excellent. After Dr. Zikira performs your laser treatment, there may be a little discoloration. After several days a crust will form and fall off within 7 - 14 days.       What is Tissue Tightening?

      Our doctor performs Tissue Tightening, a non-invasive procedure used to tighten or lift sagging skin, for patients in our region. The procedure is non-surgical and uses safe, infrared light or radio frequency to cause the collagen in the skin's dermis to contract and tighten. It can be used in all areas of the body where tighter skin is desired. The deep heating of the skin causes some immediate contraction of the collagen and additional improvement over the next 3-6 months. The optimum treatment is performed with multiple passes and moderate settings.

      Who are the Best Candidates for this Procedure?

      All patients who would like to look younger and are concerned with loose sagging skin, regardless of skin type or color, may benefit from Tissue Tightening.

      What Kind of Results Can I Expect?

      Facial skin and the skin along the neck can be tightened providing a mild, non-surgical browlift or facelift effect. This treatment is often used for sagging jowls.       Contractions of our ankle and calf muscles help to pump blood back up to our hearts. Valves inside the veins prevent the blood from falling back down in between heartbeats. As we age, the veins in our legs begin to stretch and lose their elasticity. The valves no longer function properly. This allows blood to flow backward and form pools in the weakened, lower veins. The resulting twisted, lumpy mass of blue veins bulging just beneath the surface of the skin are known as varicose veins.

      • Surgical Techniques - Snclude ligation or tying off of a vein and stripping or removal of a long segment of vein by pulling it out with a special instrument. It has been the treatment of choice for larger veins. The surgery may be performed using local, spinal or general anesthesia. Most patients return home the same day as the procedure.
      • Ambulatory Phlebectomy - Method of surgical removal of surface varicose veins through tiny incisions made in the skin, along the treated vein. The incisions are so tiny that stitches are generally not necessary. The procedure is usually performed under local anesthesia and has few complications. This procedure is often recommended in conjunction with sclerotherapy. It can minimize the side effects of sclerotherapy and enhance your results.
      • VNUS® Closure - Minimally-invasive alternative to traditional vein stripping surgery. The procedure utilizes radio frequency (RF) energy to heat the inside of the greater saphenous vein (GSV), causing it to collapse and seal shut. During the procedure, our physician inserts a tiny catheter, with an electrode tip, into the affected vein. Once in place, RF energy is delivered to the vein wall. Gradually, the catheter is withdrawn, sealing the vein along the way. Since Closure occludes veins it virtually eliminates reflux and unsightly varicose veins. The complete procedure takes about 30 - 45 minutes. After your treatment, our doctor will most probably ask you to walk, wear compression stockings and refrain from long periods of standing for a few weeks. Most patients can return to normal activities immediately.
